Periodic Survey/Quality Action Plan

WHAT IS IT?

A Quality Action Plan (of which a Periodic Survey is a type of) is a document which allows a Facility to record their quality improvement activities. It is done in response to a Full Survey. The Quality Action Plan includes all the recommendations from the most recent Full Survey Report. The Facility is then given the opportunity to document the action that it will undertake to address the recommendations, including dates by which they expect the recommendations to be addressed.

HOW LONG DOES IT TAKE?

A Quality Action Plan is completed by the Facility. How long it takes is dependent on the number of recommendations in the Full Survey Report.

HOW OFTEN IS IT DONE?

The first Quality Action Plan in the accreditation cycle is due six months after the Full Survey. A Periodic Survey (which is a Quality Action Plan, but one which also includes a Facility Self Assessment) is due 18 months after the first Quality Action Plan. The final Quality Action Plan of the accreditation cycle is due six months after the Periodic Survey.
